Salad Swap Sickness Concerns

Ordered a salad not realizing it had pork in it. Sent it back, without a single bite, for a burger. Hours later sick as a dog. I'm from out of state and have no clue how to report to this state, restaurant website not able to receive concerning emails.  | Symptoms: Diarrhea, Fever, Nausea, Vomiting
