- comments: no certified food manager on site while preparing and serving tcs food such as ham sandwich, egg sandwich, steak sandwich,etc. Chicago certificate posted on site but the said person is not on site. Priority foundation 7-38-012. No citation issued. Instructed manager that certified food manager must be on site while serving and preparing tcs food. Christopher patino certified food service manager,arrived on site at 9:30am during inspection.

- comments: no employee health policy on site. Instructed to provide. Priority foundation 7-38-010 no citation issued.

- comments: all dry bulk food container must be properly labeled with common name outside the container.

- comments: wiping cloth for wiping the prep tables,chopping board,coolers must be stored on a bucket with sanitizing solution in between uses.

- comments: 3 compartment sink not draining on all the 3 sinks and leaking on the midlle compartment sink. Instructed to sealed the hole to stop the leak and repair to have a well draining sink. Priority 7-38-030(c). No citation issued.
