- comments: observed no person-in-charge present with city of chicago certified manager certificate.
2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on site or valid documentation while (tcs foods) (nacos w/cheese, ground beef,italian beefs etc. )are being prepared/served to the customers. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present while preparing handling and or serving (tcs foods). Priority foundation violation 7-38-012, citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no written employee health policy on site for food handler working on site during the inspection. Must provide for all employees. Priority foundation violation 7-38-010,citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: found no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that kills norovirus) priority foundation violation 7-38-005,citation issued.

- comments: observed no hand soap provided at the handwashing sink at the prep/dishwashing area and inside the washroom. Instructed to provide and maintain hand soap at all handwashing sinks at all times. Priority foundation violation 7-38-030(c), citation issued.

- comments: observed no hand paper towels provided at handwashing sink at the prep/dishwashing area and inside the washroom. Instructed to provide and maintain hand papertowels at all handwashing sinks at all times. Priority foundation violation 7-38-030(c), violation consolidated with the above citation.
51. Plumbing installed; proper backflow devices
- comments: observed water leak at drainpipe underneath the handwashing sink in the prep/dishwashing area. Instructed to repair plumbing leak and maintain.
52. Sewage & waste water properly disposed
- comments: observed wastewater backing up at the dishwashing 3-compartment sink when the water is released, or run, from middle compartment, water backs up into the left and right compartments of the 3-compartment sink and water slowly draining and coming out of the grease trap box underneath said sink. Instructed to repair plumbing issue. Priority violation 7-38-030 (c), citation issued.
57. All food employees have food handler training
- comments: observed food handler(s) with no proof of food handler training certificates. Instructed to provide and maintain as required.
