Food Safety Report: 302 5th Ave, New York, NY 10001.

Ate a 6" Chicken Teriyaki and several hours later felt a fever then nausea and vomited twice. Spent the next 3 days with diarrhea, severe stomach cramps and muscle aches. I know it was the Subway because it was the only food that my girlfriend didn't eat and she did not get sick. I'm sure the chicken wasn't kept in a clean/cool environment. I'm never eating Subway again, sticking to Jimmy John's from now on. | Symptoms: Diarrhea, Nausea, Fever, Vomiting
