- comments: observed no soap at hand washing sinks in boys first(115), second((215), and third(301) floor washrooms. Instructed manager to provide soap at all hand washing sinks for proper hand washing. Priority foundation 7-38-030(c)

- comments: observed timed faucet not functioning properly at hand washing sink in 120 washroom. Water must run continuously for at least 15 seconds before reactivating faucet.
38. Insects, rodents, & animals not present
- comments: observed over 100 mice droppings scattered on floor and shelves in closet 120 and under desk in washroom 120, 5 mice droppings scattered near refrigerator in teachers lounge. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed low hot water pressure at middle sink and no hot water at right and left sinks in girls first floor washroom near main entrance. Instructed manager to repair and maintain.

- comments: observed no covered receptacle in unisex and girls washroom near 118. Instructed manager to provide covered receptacles in all girls and unisex washrooms.

- comments: observed excessive clutter in 120 closet. Instructed manager to remove all unnecessary articles and store items six inches off floor and away from walls for easy cleaning and pest control.
