16. Food-contact surfaces: cleaned & sanitized
- comments: interior of ice machine compartments and front plastic sliding doors not cleaned, beginning of mineral build-up. Ice is used for human consumption. Instructed to clean, sanitize and maintain unit.

40. Personal cleanliness
- comments: observed cook with beard and without a beard restraint. Instructed to provide beard restraints for all food handlers with beards. -

51. Plumbing installed; proper backflow devices
- comments: water leaking at faucet neck at scrape sink attached at low temp dish machine. Repair and maintain

54. Garbage & refuse properly disposed; facilities maintained
- comments: ground surrounding garbage dumpster outside need to be clean and remove items included used gloves.

55. Physical facilities installed, maintained & clean
- comments: missing floor tiles in front of ice machine. Observed broken and missing grout on floor tiles in prep,dish room some area water between floor tiles. Repair said cause. Surface must be smooth cleanable and non-absorbent material.

57. All food employees have food handler training
- comments: no food handler training certificate provided on site,instructed to obtain.

58. Allergen training as required
- comments: additional allergen certificate must be provided on site for all certified food service managers
