Food Safety Report: Domino's Pizza.

Fri Evening July 26 2013, had 2 slices of pie 1- Meat Extravaganza 1- Veggie. Within 2hrs I started with stomach cramping. Up all Fri night in intense pain, vomiting/dry heaving all night into Sat. Slight fever with intense sweating by mid morning Sat. Continued cramping & dry heaving till Sat night. Sun able to slowly hydrate & sm food. Still cramping with decreased diarrhea. | Symptoms: Diarrhea, Nausea, Fever, Vomiting
