- comments: 3-305. 14 observed a live small flying insect land on a munchkin donut in the donut storage case. Management voluntarily discarded 2 trays of donuts which were affected. Management instructed to protect all foods from contamination. Priority foundation 7-38-005. Citation issued.

- comments: 6-501. 114 must organize the rear storage area and garage. Remove any unused items and store remaining items off of the floor and away from the walls to eliminate pest harborage conditions.
59. Previous priority foundation violation corrected
- comments: 8-404. 13(b)(3) previous priority foundation violation #38 not corrected from report #2385689 on 8/4/20: 38 - observed approximately 15 live small flies on the walls, ceiling and boxes of stock in the food prep, storage areas and washroom. Additional pest control service is needed to eliminate the pest activity. Priority 7-42-090. Citation issued.
