- comments: observed open employee beverage containers stored above single-service items and non food-contact surfaces. Instructed to remove all open beverage containers and use closed beverage containers in designated employee storage areas and maintain.

- comments: observed ice shield in in-use ice machine soiled to sight and touch. Instructed to dispose of ice, clean and sanitize food-contact surface and maintain. Ice is used for human consumption priority foundation violation. 7-38-005 citation issued.

- comments: observed internal thermometers missing from front reach-in coolers (near front 3-compartment sink, near ice cream cooler). Instructed to provide internal ambient air thermometers to all cold-holding equipment and maintain.

- comments: observed in-use coffee stirrers and spoons stored in stagnant, room-temperature water. Instructed to remove in-use utensils from water and store on clean, sanitized surface in-between uses and maintain.

- comments: observed rusted components of equipment and non-food contact portions of equipment (ovens, wire shelves, cold-holdoing equipment) in poor repair. Instructed to repair or replace equipment so that non-food contact surfaces are smooth and easily-cleanable and maintain.

- comments: observed exterior non-food contact portions of ice machine and cold-holding equipment along drive-thru line to be soiled. Instructed to clean and sanitize all non-food contact surfaces at an adequate frequency to prevent soil and debris buildup and maintain.

- comments: observed paper cup used in place of condensate drainage pipe for cold-holding equipment in the rear mop sink area. Instructed to remove and replace with plumbing fixture that is smooth and easily-cleanable and maintain.

- comments: observed the following previous core violations not corrected on-site: #55 observed excessive dirt and food debris on the floors around the ovens, coolers, ice machine, and back storage areas-----floor drains under the cash register and in between cooking equipment with excessive dirt build-up. Instructed to clean and maintain. #55 observed unnecessary items on floor in dry storage area. Instructed person in charge to elevate all items and maintain. Priority foundation violation. 7-42-090 citation issued.
