High Priority - Roach activity present as evidenced by live roaches found. Approximately 14 live roaches observed on wall at dishwasher. Live roaches were gathered on blue fire alarm tag attached to pole for Ansul System. 1 live roach observed in between walk in cooler door 2 live roaches observed in corner of ware washing machine going into trim molding and into ceiling.
High Priority - Raw animal food stored over or with unwashed produce. Raw tuna poke stored above unwashed Brussel sprouts in walk in cooler. Operator reorganized.
