Report #2589902 dated february 22 2024 instructed facility to: #47 replace or repaint rusted racks in 4 door cooler, #51 install backflow prevention device on ice machine and coffee maker, and to seal large hole in ceiling in staff toilet room. Observed rusted racks in 4 door cooler; observed no visible backflow prevention device on ice maker and coffee maker; observed 2 large holes in ceiling of staff toilet room. Instructed manage to address all previous core violations. Priority foundation violation 7-42-090 citation issued.
