Informed person in charge city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving and operating hours. - comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees. Priority foundation violation #7-38-010. No citation issued.

- comments: observed two male food handlers prepping food without hair restraints. Instructed person in charge that all food handlers must wear a hair restraint.

- comments: observed an excessive amount of debris and ice buildup in the chest freezer located in the front food prep. Must remove excess ice, detail clean, and maintain.

- comments: observed a leak at the facuet of the three compartment sink. Instructed to repair.

- comments: observed excess clutter and unnecessary articles stored in the rear food prep. Must remove all unnecessary articles, clean all clutter, organize, and maintain areas at all times.
