OBSERVED TRAY OF RAW DOUGH STORED BENEATH FOOD PREPARATION TABLE NEAR DILAPIDATED WALL WITH MORE THAN 10 LIVE ROACHES.

OBSERVED UNCOVERED TRAY CONTAINING COOKED DOUGH STORED ON RACK AT BACK OF HOUSE KITCHEN AREA. COOKED FOOD ITEMS HAD MORE THAN THREE LIVE ROACHES IN FOOD ITEMS.

OBSERVED CONTAINER OF FOOD ITEM WITH TWO LIVE ROACHES.
