- comments: observed 6lbs. Of chicken quarters being cooked outside on grill in rear yard while it is raining at this time. Instructed manager to prepare foods in facility under ventilation hood only to protect food from elements. Product was discarded by manager. Appx. 6lbs. $30. (priority 7-38-005)

- comments: observed evidence of appx. 25 or more rat droppings under 2- door reach in cooler. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/14/20. (priority foundation 7-38-020(a)

- comments: must repair or replace leaky pipe at hand sink in prep area.

- comments: must repair or replace damaged floor tiles throughout prep & dish washing areas under floor mats throughout the areas. Holes in lower wall base behind 2- door reach in cooler,hole in floor near rear door and hole around wiring leading from rear wall.
