Tuesday January 16, 2018. Around 630 pm. Order shrimp on the beach with mushrooms. Started feeling stomach pains around 1130 pm. Got extremely sick around 2:30 am. Violent throwing up. Massive stomach cramps. Then two days of Diarrhea and pain. Dehydrated. Missed three days of work | Symptoms: Diarrhea, Fever, Nausea, Vomiting, Cramps, Dehydration, Stomach Pain
