- comments: observed no hand washing sign at hand sink in hallway outside of pre-k room. Must provide and maintain.

- comments: observed evidence of appx. 30 or more mice droppings on floor next to both refrigerator and file cabinet in kitchen,along wall bases and 2- door reach in cooler in rear storage area,on floor in corners of lunchroom. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/12/22. (priority foundation 7-38-020(a)(citation issued)

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no garbage can with lid in staff toilet room. Must provide and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: must provide light shields on light bulbs in lunchroom or provide shatter proof bulbs.
