- comments: observed over 100 mice droppings scattered on cement block under 3-compartment sink on floor along walls and corners under 3-compartment sink, behind stove, along cabinets in kitchen, on floor along walls and corners in kitchen storage areas, on floor along walls and corners in front staff washroom, on cubbies in hallway. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed drainpipe not connected directly to floor drain under hand washing sink in kitchen. Instructed manager to connect drainpipe directly to floor drain to prevent spillage.

- comments: observed openings along wall behind furniture in front lobby, along floor, walls, ceiling around pipe fittings in kitchen. Instructed manager to seal all openings in walls and ceilings in all areas.
