Rear storage area over 60 droppings under and behind cooler units. Instructed to clean,sanitize and maintain areas. Today 1-13-2021 still observed mice droppings throughout the premises: kitchen on top prep table by opened food, under shelves, beside ice machine and throughout storage areas. Instructed to remove all droppings, clean and sanitize equipment and surfaces. Have premises serviced by pest control company. Priority violation: 7-42-090, citation issued. Today 1-20-21 re-inspection requested by owner we still observed mice droppings in different location of premises. Over 50 rodent droppings. Also a live adult roach in the kitchen inside the microwave. Instructed to remove all droppings, clean and sanitize equipment and surfaces. Eliminate pest activity. Have premises serviced by pest control company.

This voluntary recall is due to possible undeclared allergens (egg) in Dole™ Sesame Asian Chopped Salad Kit. The dressing and topping kit (masterpack) designed for a different Dole product was unintentionally used during a portion of the production of the salad.

Dole Fresh Vegetables, Inc. is coordinating closely with regulatory officials. No illnesses or allergic reactions have been reported to date in association with the recall. However, people who have an allergy to eggs may have a serious or life-threatening allergic reaction if they consume this product or products containing eggs.

The product lot code and Best if Used by Date is located on the upper right corner of each bag and the UPC code is located on the bottom right corner of the back of each bag. It tasted bad yes but it also was not cold like it usually is. All the items, the lettuce was yellow, the other veggies, everything was off like the refrigeration is maybe saying it's working but isn't or that it's altogether just been out too long or is way too old. I stopped eating it.

Anyhow about 15, 20 minutes later we were still driving and I had to have my partner pull over so I could vomit on the side of the road for about 5 minutes. My stomach is still feeling awful. It's unacceptable.

See Moreuality Cookies and Cream Ice Cream (48 oz.), and 502 bulk containers of Klein’s Vanilla Dairy Ice Cream (3 gallon) because the products may be contaminated with extraneous material, specifically metal filling equipment parts. There has been one report of a customer who discovered an intact piece of metal equipment in the Weis Quality Cookies and Cream Ice Cream (48 oz).

There is a concern of an additional piece of equipment present in the ice cream product(s) possibly presenting a choking hazard. The Weis Quality Cookies and Cream Ice Cream (in 48 oz. containers) product has been removed from sale. It was sold in 197 Weis Markets’ stores in Pennsylvania, Maryland, Virginia, New Jersey, New York, Delaware and West Virginia.

The Weis Quality Cookies and Cream ice cream is packaged in a scround 48- ounce container with a UPC of 041497-01253 and with a sell by date of 10/28/21. This was produced on 10/28/2020 and released for sale on 10/29/2020. The sell by date is located on the bottom of the container. An example of the label has been included with this release.

Customers who have purchased this product may return it for a full refund. The Klein’s Vanilla bulk ice cream is packaged in 3-gallon containers with a code stamp of 0302 and are not for retail sale. This product was sold to one retail establishment in New York and has been removed from sale. Additional ice cream products packaged on 10/28/2020 included Klein’s Cookies and Cream Dairy Bulk – 665 containers, and Nelson’s Graham Slam Bulk – 286 containers.

These items are packaged in 3-gallon bulk containers with a code stamp of 0302 and are not for retail sale. They were stored in a warehouse and have not been distributed. FSA advises consumers not to eat the recalled product. Instead, return it to the store from where it was bought for a full refund. The product contains carrageenan which can present a choking hazard due to the consistency, solubility, shape and size, and manner of consumption of the food. The product can be projected from the cup or capsule in one piece by exerting pressure at the tip and therefore can cause choking hazard.
