Inspectors found several significant food safety violations during the inspection. There was no person in charge or food employee on site with the required City of Chicago certified food manager training and original certificate while the facility was preparing and serving time- and temperature-controlled food, including pizza. A citation was issued for the missing certified food manager requirement.

Inspectors also found that the low-temperature dish machine was in poor repair, leaking severely, and not being used. The facility did not have another way to wash, rinse, and sanitize equipment and utensils. The dish machine was tagged, and a citation was issued. In addition, no working probe thermometer or other food temperature measuring device was available on site, and a citation was issued for that violation.
