- comments: observed openings at rear exit door next to employee all gender washroom. Observed openings at base and right side of rear exit door next to employee all gender washroom. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed approximately 50 mouse droppings in facility. Observed mouse droppings on floor and in book rack in classroom to the left of front entrance door and in kitchen on floor near stove and refrigerator. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Recommended licensed pest control company service facility on or after 1/26/2026. Priority foundation violation 7-38-020(a) citation issued
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed doors of all three washrooms(all gender staff and two student washrooms) to not be self closing. Instructed to install self closing devices on all three washroom doors so that all three washroom doors shall be tight fitting and self closing.

- comments: observed damaged floor in front of rear exit door next to all gender staff washroom. Instructed to smoothly repair damaged floor in front of rear exit door next to all gender staff washroom. 0bserved opening in between floor and wall in kitchen. Observed opening in between floor and wall next to tall cabinet near ansul system in kitchen. Instructed to smoothly repair opening in between wall and floor next to tall cabinet near ansul system in kitchen for ease of cleanability.
