During a Nov. 19 health inspection, officials found four high-priority violations.

The report said 10 live flying insects were found in a prep area landing on walls and clean utensils, 50 live flying insects were found at the cook line. Also, inspectors said they found flies “landing on single service items, soda boxes and containers of food.” Also, a live roach was found on a wall above a sink.
