- comments: observed no handwashing signage at handsinks in boys and girls' restrooms near gym. Instructed person in charge to provide handwashing signage in all restrooms at all times.

- comments: observed no hot water at the following sinks on premises with temperatures from 65. 0f-72. 3f; two compartment sinks in kitchen, handsink in kitchen, 1st floor boys and staff restroom, 2nd floor boys and girls' restroom. (hot water observed in the girls' restroom on the first floor with a separate hot water tank). Instructed person in charge to restore hot water to minimum of 100f per city requirements. Priority violation #7-38-030(c). Citation issued.

- comments: observed multiple holes in walls throughout facility where access doors were removed. Observed peeling paint on walls in hallway of stairwell. Instructed person in charge to repair all areas and maintain smooth and easily cleanable.

- comments: observed missing and severely water damaged tiles in ceiling of gym. Instructed person in charge to repair and maintain.

- comments: observed the previous core violations from report #2589625, dated 2/25/24, not corrected as follows; #38-observed a 1/4" gap at the bottom of the rear exit door located in the basement. Must provide a tight-fitting seal at all outer openings to prevent pests ' entry. #51-observed two of four out-of-order urinals in the boys' toilet room. Also, i observed the left-hand sink in the girl's toilet room faucet spray out. Priority foundation #7-42-090. Citation issued.
