Details: OBSERVED NO EMPLOYEE HEALTH POLICY ON THE PREMISES. MANAGEMENT INSTRUCTED TO HAVE A SIGNED EMPLOYEE HEALTH POLICY FROM EACH EMPLOYEE. PRIORITY FOUNDATION 7-38-010. CITATION ISSUED.
OBSERVED NO CLEAN UP PROCEDURE AND SUPPLIES ON THE PREMISES TO RESPOND TO VOMIT AND DIARRHEAL EVENTS. MANAGEMENT INSTRUCTED TO PROVIDE BOTH. PRIORITY FOUNDATION 7-38-005. CITATION ISSUED.
OBSERVED IMPROPER COLD HOLDING TEMPERATURES OF TCS FOODS INSIDE OF THE 2 DOOR PREP COOLER. OBSERVED CHICKEN AT 46.4F, PASTA SALAD AT 47.1F, BEEF AT 57.1F, CONSUME AT 50.7F. MANAGEMENT VOLUNTARILY DISCARDED APPROXIMATELY 50# OF FOOD WORTH $100. PRIORITY 7-38-005. CITATION ISSUED.
MUST PROVIDE A THERMOMETER INSIDE OF THE 3 DOOR PREP COOLER.
OBSERVED APPROXIMATELY 30 RAT DROPPINGS ON THE FLOOR IN THE BASEMENT STORAGE ROOM. MANAGEMENT INSTRUCTED TO CLEAN AND REMOVE ALL RODENT DROPPINGS FROM THE BASEMENT. ALSO OBSERVED 10 LIVE SMALL FLIES ON THE EQUIPMENT IN THE REAR FOOD PREP AREA. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE ALL PEST ACTIVITY. PRIORITY FOUNDATION 7-38-020(A). CITATION ISSUED.
OBSERVED A KNIFE STORE IN BETWEEN THE PREP TABLE AND PREP COOLER. MANAGEMENT INSTRUCTED TO PROPERLY STORE ALL KNIVES ON A KNIFE RACK.
CLEAN THE PREP TABLES, STORAGE SHELVES THROUGHOUT, STEAM TABLE, GRILLS AND OVEN.
OBSERVED NO HOT WATER AVAILABLE THROUGHOUT THE PREMISES. THE HOT WATER WAS 61F AT THE 3 COMPARTMENT SINK. MANAGEMENT INSTRUCTED TO PROVIDE HOT WATER ABOVE 110F AT THE 3 COMPARTMENT SINK AND ABOVE 100F AT ALL HANDWASHING SINKS. PRIORITY 7-38-030(C). CITATION ISSUED.
MUST PROVIDE A COVERED WASTE RECEPTACLE IN THE EMPLOYEE WASHROOM.
CLEAN THE FLOOR THROUGHOUT THE BASEMENT. CLEAN THE WALL BEHIND THE COOKING EQUIPMENT AND IN THE EMPLOYEE WASHROOM AND ABOVE THE COOKING LINE.

OBSERVED AN ACCUMULATION OF FOOD DEBRIS INSIDE THE FRYER CABINETS. MUST DETAIL CLEAN AND MAINTAIN. 55 - 6-201.15: **C** Floor Covering, Mats and Duckboards OBSERVED CARDBOARD BEING USED AS FLOOR MATS THROUGHOUT THE FOOD PREP AREA. MUST REMOVE AND PROVIDE ALL FLOOR MATS TO BE A NON-ABSORBENT, NON-CORROSIVE, SMOOTH, EASILY CLEANABLE MATERIAL. 55 - 6-501.11: **C** Maintained in Good Repair, Facilities (FACILITIES NOT IN GOOD REPAIR) OBSERVED STAINED AND MISSING CEILING TILES THROUGHOUT FOOD PREP AREA. MUST REPLACE AND MAINTAIN. 55 - 6-501.12: **C** Cleaning, Frequency and Restrictions OBSERVED EXCESS FOOD DEBRIS ENCRUSTED ONTO THE FLOOR UNDER THE COOKING EQUIPMENT AND UNDER THE 3 COMPARTMENT SINK. MUST DETAIL CLEAN AND MAINTAIN. 56 - 6-501.14: **C** Cleaning Ventilation Systems, Nuisance and Discharge Prohibition OBSERVED AN ACCUMULATION OF DIRT/DEBRIS BUILDUP ON THE TOILET ROOM EXHAUST VENTILATION. MUST DETAIL CLEAN AND MAINTAIN. 57 - 2-102.13: **C** Food Handler Training (PA 100-0367 Food Handling Regulation Enforcement Act) MUST PROVIDE VALID FOOD HANDLER CERTIFICATES FOR ALL EMPLOYEES. 58 - PA 100-0367: **C** Allergen Training - All certified food service sanitation managers employed by a restaurant must receive or obtain training in basic allergen awareness principles within 30 days after employment and every 3 years thereafter. Training programs must be accredited by the American National Standards Institute or another reputable accreditation agency under the ASTM International E2659-09 (Standard Practice for Certificate Programs).
