- comments: observed improper temperatures of 1 pan of approximately 2 lbs of sliced lettuce at 64. 0f on prep table. Product was discarded by manager. Must have cold holding foods at 41. 0f or below. (priority 7-38-005) no citation issued

24. Time as a public health control; procedures & records
- comments: observed that facility use time temperature control for safety food but manager is unable to present written procedure prepared in advance. Must have written procedure for time temperature control. Priority foundation violation. 7-38-005 no citation issued

40. Personal cleanliness
- comments: noted employee in food prep with no hair restrain working with exposed food. Instructed to wear proper hair restrain at all times when preparing food and working with exposed food

43. In-use utensils: properly stored
- comments: observed that ice scoop is stored on top machine with out any protection. Must stored utensils under running cold water, under hot water (135 f and above),in food handle not touching food and in clean place protected from contamination.

- comments: observed broken gasket on walk in freezer door and broken air curtain on walk in cooler door

- comments: must replace or repair an maintain caulking on three compartment sink connection to wall that is covered with black slimy substance.

- comments: must clean or replace stained and broken ceiling tiles in basement storage area

Provide and maintain a back flow device on the ice machine and maintain.
