- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

23. Proper date marking and disposition
- comments: observed no date markings on tcs ready to eat foods being held for more than 24 hours inside of coolers. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed no probe thermometer for taking food temperatures. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed evidence of appx. 40 or more mice droppings on floor under all food storage shelves and on wood ledge in meter closet in lobby. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 3/26/20. (priority foundation 7-38-020(a)

- comments: must provide a splash guard at hand sink in rear prep / dish washing area next to clean food storage buss pan or remove buss pans from area.

- comments: must remove cardboard from lower shelf of prep table.

- comments: must clean debris build up from walk in cooler fan guard covers,fryer cabinet.

- comments: must repair or replace damaged wall bases near both hand sinks in prep area. Stained ceiling tiles in rear ,prep and lobby areas. Must remove excessive caulking along wall in rear area.

- comments: must replace burnt out light at hood of cooking equipment.

- comments: observed no food allergen training for all food managers. Must provide and maintain.
