Details: OBSERVED RE FRIED BEANS BEING HELD AT IMPROPER TEMPERATURES 126.3F AND BLACK BEANS AT 79.0F IN THE STEAM TABLE. MANAGEMENT DISCARDED AN ESTIMATED 2 POUNDS OF BEANS VALUED AT $1.60. INSTRUCTED TO MAINTAIN ALL HOT HOLDING TEMPERATURES ABOVE 135F. PRIORITY VIOLATION 7-38-005, CITATION ISSUED.
OBSERVED IMPROPER COLD HOLDING TEMPERATURES IN COOKS LINE ONE DOOR COOLER CHICKEN AT 62.6, PORK 63.0, AND BEEF AT 60.8. MANAGEMENT DISCARDED APPROXIMATELY 20 POUNDS WITH AN ESTIMATED VALUE OF $70. PRIORITY VIOLATION 7-38-005. SEE VIOLATION #21 FOR CITATION.
OBSERVED COOKED CHICKEN IMPROPERLY DATE MARKED WITH THE PREPARATION DATE ONLY AND OTHER ASSORTED COOKED MEATS WITH NO LABELS IN SEVERAL COOLERS. INSTRUCTED ALL REFRIGERATED, PREPARED READY-TO-EAT, TCS FOODS MUST BE LABELED WITH THE EXPIRATION DATE NO LONGER THAN 7 DAYS. PRIORITY FOUNDATION VIOLATION, 7-38-005. CITATION ISSUED.
OBSERVED ONE DOOR COOKS LINE COOLER MAINTAINING AN IMPROPER AMBIENT AIR TEMPERATURE OF BETWEEN 53.F AND 53.8F. WITH FOOD SUCH AS COOKED CHICKEN AT 62.6F. COOLER TAGGED AND HELD FOR INSPECTION AND MUST NOT USE UNTIL REPAIRED, MAINTAINS A TEMPERATURE OF 41F OR BELOW, C.D.P.H. CONTACTED FOR TAG REMOVAL. PRIORITY VIOLATION 7-38-005, CITATION ISSUED.
OBSERVED FRONT ENTRY DOOR PROPPED OPEN. MANAGEMENT CLOSED DOOR. INSTRUCTED EXTERIOR DOORS LEFT OPEN FOR AIR CIRCULATION MUST HAVE A SCREEN TO PREVENT PEST INVASION.
OBSERVED NO SPLASH GUARDS ON BOTH SIDES OF THE HAND WASHING SINK IN REAR DISH/PREP AREA BETWEEN THE THREE COMPARTMENT CLEAN DISH DRYING RACK AND THE ONE COMPARTMENT PREP SINK. INSTRUCTED TO INSTALL.
OBSERVED LEAK ON MOP SINK NOZZLE. INSTRUCTED TO REPAIR AND MAINTAIN.
OBSERVED EXPANDING SPRAY FOAM USED AS SEALANT AROUND PIPING ON FLOORS, WALLS, AND CEILINGS. INSTRUCTED TO REMOVE TO MAKE SMOOTH, CLEANABLE SURFACE.
WALL IN WASHROOM IN POOR REPAIR WITH MISSING PLASTER. MUST REPAIR TO BE SMOOTH AND CLEANABLE.
MISSING VENTILATION IN EMPLOYEE RESTROOM. DUCT TAPE COVERING CEILING WHERE VENTILATION WAS REMOVED. INSTRUCTED TO REPLACE AND REMOVE ALL DUCT TAPE.
OBSERVED UN-SHIELDED BULB IN REAR PREP AREA. INSTRUCTED TO REPLACE WITH SHATTER PROOF LIGHT BULB OR PROVIDE SHIELD.
PREVIOUS CORE VIOLATIONS FROM 8/21/18 INSPECTION #2213262 NOT CORRECTED. #37-ALL BULK FOOD CONTAINERS MUST BE PROPERLY LABELED. #55 ITEMS STORED AT REAR OF THE KITCHEN EXIT DOOR BY THE CHEST FREEZER MUST BE ELEVATED 6' ABOVE THE FLOOR AND ORGANIZED. ALL UNNECESSARY ITEMS MUST BE REMOVED FROM THE PREMISES.-#58 NO PROOF OF FOOD ALLERGEN TRAINING OR CERTIFICATES FOR ALL CITY OF CHICAGO CERTIFIED FOOD MANAGER.
