- comments: observed no vomit and diarrhea clean up kit or procedure on site. Must provide. Priority foundation violation 7-38-005. Citation issued.

- comments: **establishment serving ceviche, eggs, steak and hamburgers that are cooked to order. Instructed to provide a consumer advisory on the menu with a disclosure notice and reminder statement. Provide an asterik on the menu of foods that are raw/cooked to order. Priority foundation violation 7-38-005 citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed dry storage items (canned soda) stored directly under waste water pipes in basement. Must relocated storage items, or completely encase waste water pipes to prevent contamination.

- comments: observed small amounts of brown/orange slimy mold like substance inside ice machine. Must remove, clean and sanitize. Must maintain same.

- comments: observed previous core violations not corrected from report # 2313186, dated 10/1/19. Must correct the following previously issued core violations: #10 observed no employee hand washing signs in bathrooms. Instructed manager to provide. #37 observed spices out of original containers not labeled. Instructed manager to label food ingridient containers with contents name. #53 observed no lid on trash can in women's bathroom. Instructed to provide. #58 no allergen training certificate on site during inspection. Instructed manager that employees who have the city of chicago sanitation certificate, must provide allergen certificate. Priority foundation violation 7-42-090. Citation issued.
