- comments: observed no certified food manager on duty and no valid certificate posted to view while tcs foods are being prepared,handled and served such as chicken,steak,gyros,etc. ) must be on site at all times. - comments: observed no hand washing sink in rear slushie prep area. Must install a hand sink with hot and cold running water or move prep area near additional hand sinks on site. (priority foundation)

- comments: observed evidence of appx. 100 or more mice droppings on floor under slushie prep table,on floor behind small chest freezer,behind bread rack,next to furnace,under glass & 3- door storage reach in cooler,all fryer cabinets and meat slicer on dry food storage shelf. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 2/20/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '1/2-3/4' gap along bottom of both rear doors and front south east door in corner. Must make doors tight fitting.
