- comments: the pic has no certified food managers certificate. Must provide and maintain. (priority foundation)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty and no valid certificate posted to view while tcs foods are being prepared,handled and served such as chicken,steak,gyros,etc. ) must be on site at all times. Manager arrived on site at 3:00 p. M. With valid certificate. (priority foundation 7-38-020)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing sink in rear slushie prep area. Must install a hand sink with hot and cold running water or move prep area near additional hand sinks on site. (priority foundation)

- comments: observed improper temperature of appx. 30lbs. Gyros on spit at rotesserie at 55. 8f. Product was discarded by manager. Must have hot holding foods at 135. 0f or above. (cos). 30lbs. $75. (priority 7-38-005)

- comments: observed evidence of appx. 100 or more mice droppings on floor under slushie prep table,on floor behind small chest freezer,behind bread rack,next to furnace,under glass & 3- door storage reach in cooler,all fryer cabinets and meat slicer on dry food storage shelf. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 2/20/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '1/2-3/4' gap along bottom of both rear doors and front south east door in corner. Must make doors tight fitting.

and headache wouldn’t stop all night, I would wake up out of my sleep with a mouthful of vomit so i had get a bag to sleep beside, this all came with severe stomach pain, severe headache, and puking with every breathe i took until eventually all i was puking was stomach acid. Tried to drink water and i puked that right up. I woke up needing to use the bathroom and it’s been complete liquid and it’s a whole day later. I still haven’t ate anything, but I am now able to hold water down.The pain caused my to sleep/lay in bed for over 24hours. At 4:30am, I was awakened with stomach cramps and ran to the bathroom. I first vomited and then the diarrhea came. Went back to bed, only to be awakened two hours later. Seven of the eight cases with available information reported exposure to fresh organic basil purchased from Trader Joe's before falling ill. Further traceback data collected by the FDA pinpointed Infinite Herbs, LLC, in Miami, FL, as the supplier of the organic basil sold at Trader Joe’s stores. Infinite Herbs, LLC is cooperating with the FDA investigation and has agreed to initiate a voluntary recall.

The product, Infinite Herbs-brand organic basil, is packaged in 2.5-oz clamshell containers and has been removed from Trader Joe's store shelves and this product should no longer be available for sale. Symptoms of Salmonella infection typically manifest within 12 to 72 hours of consuming contaminated food and can last up to a week.

Consumers who purchased organic basil from Trader Joe’s stores in the states listed above should check their refrigerators and freezers and should not eat 2.5-oz packages of Infinite Herbs-brand organic basil.
