- comments: observed reach in cooler not maintaining proper temperature 49. 2f. Instructed manager to repair and maintain cooler at 41f or below. Prep cooler is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005.

- comments: observed open bait trap located in storage area. Manager removed bait during inspection.

- comments: observed leaky pipe at 3 compartment sink in kitchen. Engineer on site repaired sink at time of inspection.

- comments: observed no covered receptacles in staff/gender neutral and girls restroom. Instructed to provide and maintain.

- comments: observed the outside dumpsters overflowing with garbage. Instructed to make sure all lids stay tight fitting and do not overflow outside dumpsters with garbage. Priority foundation 7-38-020(b).
