Report date: Jul 13, 20201. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge with chicago food sanitation certificate on site. Instructed manager to designate person in charge during all hours of operations. Priority foundation violation.
2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site nor city of chicago certified posted while time temperature control for safety foods are being served such as ice cream. Management instructed that a certified food manager must be present at all times tcs food is being prepared and served. Priority foundation violation 7-38-012. Citation issued
5. Procedures for responding to vomiting and diarrheal events
- comments: the person in charge unable to provide procedure for employee to follow when responding to vomiting and diarrhea events. Instructed to provide procedure with the necessary items gloves, masks, disposable mop and appropriate sanitizer. Priority foundation violation 7-38-005 citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs or posters near and washing sinks in employee/customer washroom. Must provide and maintain.
41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on top of prep tables. Must properly store wiping cloths in a sanitizing solution when not in use.
52. Sewage & waste water properly disposed
- comments: inadequate waste water disposal observed excessive sewage and waste water backing up from grease trap located underneath 3-compartment sink in food preparation area when water is draining from 3-compartment sinks. Noted strong sewage smell coming from grease trap must repair grease trap and maintain. Priority violation 7-38-030(c).
55. Physical facilities installed, maintained & clean
- comments: observed dust build up on ceiling light shields covers in food preparation area. Must clean and maintain.
