OBSERVED NO EMPLOYEE HEALTH POLICY/TRAINING ON SITE. INSTRUCTED FACILITY TO ESTABLISH AN APPROPRIATE EMPLOYEE HEALTH POLICY/TRAINING SYSTEM AND MAINTAIN WITH VERIFIABLE DOCUMENTS ON SITE. PRIORITY FOUNDATION VIOLATION 7-38-010. NO CITATION ISSUED.
OBSERVED NO PROCEDURE/PLAN FOR RESPONDING TO VOMITING AND DIARRHEAL EVENTS. INSTRUCTED FACILITY TO DEVELOP AND MAINTAIN A PROCEDURE /PLAN AND TO MAINTAIN ANY APPROPRIATE SUPPLIES ON SITE. PRIORITY FOUNDATION VIOLATION 7-38-005 NO CITATION ISSUED.
OBSERVED EMPTY HANDWASHING SOAP CONTAINERS AT ALL HANDSINKS INCLUDING WASHROOMS BEING FILLED WITH DISH SOAP AND WATER. WATERY DISHSOAP LIQUID WAS INADEQUATE TO WASH HANDS PROPERLY. MANAGER WAS INSTRUCTED TO PURCHASE PROPER HANDWASHING SOAP DURING INSPECTION PRIORITY FOUNDATION VIOLATION 7-38-010.
OBSERVED 6 BOXES CONTAINING APPROXIMATLY 60 DOZEN RAW EGGS STORED AT ROOM TEMPERATURE IN KITCHEN. EGGS WERE FOUND AT 62.8 F WEIGHING APPROXIMATELY 150LBS VALUED AT $200. EGGS WERE VOLUNTARILY DISCARDED AND DESTROYED. PRIORITY VIOLATION 7-38-005.
OBSERVED NO DATE MARKING SYSTEM IN PLACE. READY TO EAT AND OR TIME TEMPERATURE CONTROLLED FOODS PREPARED AND HELD IN A RETAIL FOOD ESTABLISHMENT FOR OVER 24 HOURS MUST HAVE A CLEARLY MARKED LABELING SYSTEM IN PLACE TO INDICATE THE DATE OR DAY BY WHICH THE FOOD SHALL BE CONSUMED ON PREMISES, SOLD, OR DISCARDED WHEN HELD AT A TEMPERATURE OF 41F OR LESS FOR A MAXIMUM OF 7 DAYS WITH THE DATE OF PREPARATION COUNTING AS DAY 1. PRIORITY FOUNDATION VIOLATION 7-38-005 NO CITATION ISSUED.
OBSERVED APPROXIMATELY 50 MICE DROPPINGS IN FRONT PREP AREA ALONG WALLS UNDER AND BEHIND COOKING AND COOLING EQUIPMENT. MUST PULL ALL EQUIPMENT AWAY FROM WALLS AND REMOVE ALL MICE DROPPINGS. MUST CONTACT PEST CONTROL FOR A RECENT PEST CONTROL SERVICE. PRIORITY FOUNDATION VIOLATION 7-38-020(a).
OBSERVED TORN GASKETS ON 2 DOOR REACH IN REFRIGERATOR WITHIN FOOD PREP AREA. MUST REPLACE TORN GASKETS.
OBSERVED REACH IN CHEST FREEZERS UNCLEAN IN BASEMENT. ALL REACH IN CHEST FREEZERS NEED TO BE CLEANED IN DETAIL. OBSERVED BEER COOLER ON 2ND FLOOR BAR UNCLEAN WITH STANDING WATER IN BOTTOM. COOLER NEEDS TO BE CLEANED AND MAINTAINED.
OBSERVED FROM PREMISES AND ALL REMAINING ITEMS NEED TO BE STORE PROPERLY AWAY FROM WALLS AND UP OFF GROUND IN ORDER TO MONITOR FOR PEST ACTIVITY AND CLEAN THOROUGHLY.
ALL MAINTENANCE TOOLS NEED TO BE STORED IN A DESIGNATED AREA AWAY FORM FOOD.
ALL COVING NEEDS TO BE REPLACED IN REAR NEAR REAR EXIT DOOR BY 3 COMPARTMENT SINK. REPLACE COVING AND MAINTAIN FLOOR/ WALL JUNCTURE.
OBSERVED FLOORS VERY UNCLEAN UNDER HOT HOLDING UNITS IN FRONT PREP AREA. FLOORS NEED TO BE CLEANED IN DETAIL AND MAINTAINED. OBSERVED FLOORS UNDER 3 COMPARTMENT SINK AND TO THE LEFT OF 3 COMPARTMENT SINK UNDER RACK VERY UNCLEAN. MUST CLEAN AND MAINTAIN ALL FLOORS THROUGHOUT FRONT AND REAR FOOD PREP AREAS.
OBSERVED CEILING OF WALK IN FREEZER PEELING AND CHIPPING. MUST REMOVE ALL PEELING PAINT AND RESTORE SURFACE TO A SMOOTH EASILY CLEANABLE CONDITION AND MAINTAIN.
MUST PROVIDE PROOF OF FOOD HANDLER TRAINING FOR ALL EMPLOYEES.
OBSERVED NO APPROVED ALLERGEN TRAINING IN ACCORDANCE WITH PA 100-0367 ON SITE. INSTRUCTED ANY PERSONS WITH THE CITY OF CHICAGO FOOD SANITATION CERTIFICATE TO COMPLY WITH ALLERGEN TRAINING AND MAINTAIN.
