- comments: observed excessive debris build up on equipment. Must detail clean and sanitize all equipment. Observed feather present on equipment in prep area. Must remove. Observed heavy debris on bird cages. Must remove.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed toilet overflows when flushed. Which comes out from the bottom of the toilet on to the floor. Must repair. Priority foundation violation 7-38- no citation issued. 7-38-030c
55. Physical facilities installed, maintained & clean
- comments: floor in the prep areas, rear storage area, under the chicken cage, and outside the exit door with bird's feathers, blood and bird's feces and debris. Instructed to detail clean, and sanitize the areas and maintain. Floor in the front serving needs detail cleaning also. Walls and ceiling behind the bird's cage not clean, instructed to detail clean,sanitize and maintain. Observed heavy debris on bird cages. Must remove.
61. Summary report displayed and visible to the public
- comments: observed no inspection summary report posted. Instructed must post most recent inspection report summary in plain view of customers. Priority foundation violation 7-42-010(b).
63. Removal of suspension sign
- comments: notice of closure/license suspended sign conspicuously posted at front door. Observed 'closed for construction' sign covering the city of chicago sign. Instructed owner to remove. Instructed to not move, remove,alter or conceal sign. Priority foundation violation 7-42-050. No citation issued.
