- comments: 5-202. 12 (b-d)--observed two (2) metered faucets at hand sinks in boys bathroom on 2nd floor running for less than 15 seconds. Repair or replace to ensure continuous flow of water for 15 seconds is provided.

- comments: 6-202. 15-- observed exterior doors of main entrance to 001 and 002 have gaps at base in center. Provides access to exterior. Instructed to pest proof and seal exterior doors from pests, and to maintain.

- comments: 6-201. 11--observed peeling paint debris and walls and ceilings in disrepair in the following locations: on ceilings above cafeteria tables in the main dining hall. On ceilings and walls in the 2nd floor girls bathroom, 2nd floor boys bathroom, 3rd floor girls bathroom. Walls of dry storage area on first floor repair all surfaces to be smooth and easily cleanable.

- comments: previous priority foundation violations #38,#54 not corrected from inspection report#2590919 dated: 03/13/2024 #38--6-501. 111:continue to observe evidence of pest activity on the premises. Observe approximately 10 rodent(mice)droppings on the floor behind and in between storage cabinets stored along the windows and 20 rodent(mice)on floors behind and around stored boxed equipments,toys and underneath cluttered cabinet in same connecting storage room between both pre-k classrooms in (outside modular pre-classrooms#001,#002). Observed approximately 30 rodent(mice)droppings in wall heaters and on top of book shelfs in same area in (outside annex pre-k classroom#131) and approximately 45 rodent(mice)droppings on floors and corners in the following cluttered areas electrical room and boiler room located next to annex pre-k classrooms #128 and #130. Instructed to detail clean remove cluttered conditions and remove all rodent droppings from all affected areas in detail. Additional pest control service is needed to eliminate the pest activity and maintain. Priority violation 7-42-090. #54--5-501. 15: observed exterior receptacles overflowing with trash bags, containers with lids not closed and covered, and loose debris on ground surrounding dumpsters. Instructed to maintain area, detail clean, and properly cover all dumpsters with tight fitting lids to eliminate potential for pests. Priority violation 7-42-090.
