1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic doesn't have a certified food managers certificate. Must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs ready to eat foods are being prepared,handled and served such as (italian beef,pre-cooked chicken,ground beef,sliced gyros)must be on site at all times. Food manager arrived on site after inspection was conducted at 11:23 a. M. (cos)(priority foundation 7-38-012)(citation issued)

22. Proper cold holding temperatures
- comments: observed improper temperature of 4 lbs. Pre-cooked chicken wings at 70. 9f in a buss pan inside of 2- door glass reach in cooler. Product was discarded by manager. Must have cold holding foods at 41. 0f or below. Appx. 4lbs. $15. (cos) (priority 7-38-005)(citation issued)

- comments: must label food storage containers (squeeze bottles,shakers, bulk,food bins)when food is not in original package.

39. Contamination prevented during food preparation, storage & display
- comments: observed plastic bins with food and buckets stored on floor in walk in cooler must store off of floor onto shelving units to prevent cross contamination.

41. Wiping cloths: properly used & stored
- comments: observed wiping cloth on topside of prep cooler must store in a sanitizing solution when not in use.

45. Single-use/single-service articles: properly stored & used
- comments: must store plastic wear with handles in upright position.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must eliminate crates used for food storage in walk in freezer and rear food storage area.

49. Non-food/food contact surfaces clean
- comments: must clean grease and food debris build up from interior and exterior of fryers,prep coolers, reach in coolers ,grill table, prep tables,rolling carts,chip display racks, service counter shelving units,meat slicer,window sills.

55. Physical facilities installed, maintained & clean
- comments: must repair or replace damaged floor tiles,missing ceiling tiles,loose wall bases in rear storage area,rear prep/dish washing area-damaged floor tiles,walk in cooler- loose threshold. Must remove excessive caulking from along wall bases throughout to be smooth and easily cleanable. Must seal hole in wall at rear door.

- comments: must clean grease and food debris build up and splatter from walls in prep/dish washing areas. Dust build up on upper wall in toilet room.
