1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no pic in charge with the chicago food sanitation certificate. Instructed to have a pic on site with the sanitation certificate at all times.

2. City of chicago food service sanitation certificate
- comments: observed no chicago certified food manager (cfm) on site while preparing/serving tcs foods (i. E. Chicken, fish, gyros, etc. ). Instructed to have one on site at all times during preparing/serving of tcs foods. Priority foundation 7-38-012 citation issued

40. Personal cleanliness
- comments: observed employees and food handlers in the food prep areas not wearing a hair restraint. Instructed to have all in the food prep areas to wear a hair restraint. Maintain same.

49. Non-food/food contact surfaces clean
- comments: observed the rubber gaskets and the lower interior surfaces of the front prep coolers with dirt and food build up. Instructed to detail clean and maintain all.

55. Physical facilities installed, maintained & clean
- comments: observed the top of the walk in cooler/freeze with clutter. Instructed to remove unnecessary items and organize the area. Maintain a clutter free area.

- comments: observed the mops in the rear not properly stored. Instructed to store hanging up for proper air drying.

- comments: observed some water stained ceiling tiles in the rear and toilet room. Instructed to replace tiles and maintain.

- comments: observe the ceiling tiles in the food prep area with dirt and dust build up. Observe the floors in the prep area under the mats and inside the walk in freezer with ice build up and food debris. Instructed to clean and maintain ceiling tiles and floor.

- comments: observed the only allergen training certificate posted expired. Instructed to have all cfms obtain and maintain a current allergen training certificate.

60. Previous core violation corrected
- comments: observed the core violations from report #2585149 dated 11/14/2023, not corrected and/or maintained. Instructed to correct and maintain the following: #37-observed two bulk dry storage containers in the front food prep area and the rear food prep area not labeled with common name. Instructed to label all dry ingredients not in the original packaging with common name on all storage containers and maintain. #49-observed the lower compartment of all of the deep-frying untis, under the steam table, the shelving unit under the bun toaster next to the steam table, and the hot holding unit above the prep cooler with the vegetables to dress ready to eat foods all found in the front food prep area; with severe grease build up, food build up, and not maintained. Instructed to deep clean all mentioned equipment and maintain. #51-observed the floor drain under the three compartment sink with a slow drain. Instructed to repair said slow drain and maintain. #53-observed the toilet in the rear employee toilet room not clean. Instructed to clean and maintain the entire toilet. #55-observed the floors under and surrounding the steam table area at the front food prep area with grease build up and food debris. Instructed to clean said floors and maintain. Priority foundation 7-42-090 citation issued
