- comments: observed evidence of appx. 60 or more live small flies and 10 or more live large flies on boxes,shelving units,walls and flying around in rear dish washing area,prep areas,staff and customers toilet rooms. Recommended to have a pest control operator to service premises on or after 8/11/20. (priority foundation 7-38-020(a)

- comments: (a) in addition to the health steps required by executive order no. 2020-43, businesses must take the following additional public health measures to protect their employees, their customers, and all others who come into physical contact with their operations: (1) maintain social distancing on the premises; observed no designated social distance floor markings in lobby for customers. Must provide and maintain.

- comments: (a) in addition to the health steps required by executive order no. 2020-43, businesses must take the following additional public health measures to protect their employees, their customers, and all others who come into physical contact with their operations: (3) ensure that every employee who interacts with customers and is able to medically tolerate a mask is wearing a mask for the duration of a shift; observed 2- male employees wearing a face mask underneath their chin and not covering nose and mouth while taking orders and handling money at front window and drive-thru with customers. Must wear mask properly and maintain.
