Observed pizza delivery drivers/employees entered establishment from the back door handling boxed, cooked pizza no hand wash. Employees washed hands. **Corrected On-Site**
High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed 3 live small flying insects flying around the front reach in cooler. Observed 1 fly landed on 2 pizza dough that was being made.
High Priority - Potentially hazardous (time/temperature control for safety) food hot held at less than 135 degrees Fahrenheit or above. Observed pepperoni pizza at 123°F on the hot holding unit. Manager discarded pizza. **Corrected On-Site**
High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live roach crawling on the floor by pizza box station, and 2 live roaches crawling behind the reach in cooler that has food.
High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 100+ rodent droppings found behind the pizza dough station, approximately 30+ rodent droppings in the storage room, approximately 10+ rodent droppings in the dining area.
High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed 1 fly landed on 2 pizza dough that was being made.
