- comments: observed food storage containers without labels containing the common name of foods. Instructed to provide.

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approximately 25 mouse droppings on the floor of the basement dry storage area on the side of the hvac unit and behind the ice chest freezer. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain them at all times.

- comments: observed grease buildup on cooking equipment, walls, surrounding floors, and shelving units in the kitchen/prep areas. Instructed to degrease, clean, and sanitize.

- comments: observed missing and damaged floor tiles in the bar and dining room. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

Instructed to detail clean under all shelving units, interior of prep coolers, hot-holding units, prep tables and deep fryers to remove spills, food debris, dirt buildup and/or grease. Must maintain at all times.

- comments: observed dust buildup on fan in walk-in cooler, light shields in cooking area, ceiling, and wall vents in prep and storage areas. Instructed manager to clean and maintain all fans, light shields, and ceiling/wall vents.
