- comments: observed evidence of appx. 40 or more droppings on floor next to church & daycare refrigerators,under 3- compartment sink and in furnance room inside of crate. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 12/7/20. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap in center of front door. Must make door tight fitting.

- comments: must repair or replace faucets in poor repair in bot's toilet room 2nd hand sink.

- comments: must elevate,remove or organize articles off of floor and away from walls in class room #2 & 3,prep area & food storage closets,furnance room. Must remove articles from inside of mop sink.
