All retail food establishments must maintain a log book which shall be kept for a period of twelve (12) months and include a copy of the service tickets and the pest control company’s illinois department of public (idph) health license number and at least 2 pest inspections a year and the reports must be maintained in the log book. Instructed to provide. Priority foundation violation #7-38-020(c).
