- comments: we observed open toxic bait for pest control improperly used( exposed on floor by stove in kitchen) instructed rodent bait must be applied inside a covered tamper-resistant trap) by licensed pest control company. Pic removed and discarded toxic bait-cos. Priority violation issued 7-38-015.

- comments: observed no chemical ( chlorine)sanitizing test kit for sanitizer being used for dishwashing facilities at 3- compartment sink. Instructed to provide. Priority foundation issued 7-38-005
