- comments: observed exterior of storage cabinet doors not clean of debris. Instructed to clean/ maintain.

- comments: observed no covered receptacle in staff east washroom. Instructed all washrooms usedd by females must have a covered receptacle for sanitary napkins. Must provide.

55. Physical facilities installed, maintained & clean
- comments: observeed opening/ hole in lower wall corner in furnace room by kitchen. Must seal to prevent hiding areas for pest.
