- comments: evidence of improper pest control on site. Observed 45 mice droppings on floors in both furnace rooms. Instructed to clean/ sanitize areas. Recommed pest service. Priority foundation 7-38-020(a).

- comments: observed exterior of storage cabinet doors not clean of debris. Instructed to clean/ maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in staff east washroom. Instructed all washrooms usedd by females must have a covered receptacle for sanitary napkins. Must provide.
55. Physical facilities installed, maintained & clean
- comments: observeed opening/ hole in lower wall corner in furnace room by kitchen. Must seal to prevent hiding areas for pest.
