- comments: must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross-contamination. Instructed to maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: replace broken cover for receptacle inside the washroom

55. Physical facilities installed, maintained & clean
- comments: observed cracked and broken floor tiles in rear prep in front of 3 compartment sink. Replace and maintain.

- comments: found no written protocol available on site. Instructed to have a written record describing protocol for checking proof of vaccination in public places. Violation 2-112-340 no citation issued

- comments: observed no signage posted at the accessible entrance to the covered location and at least one location inside premises that is conspicuously visible, informing patrons of the full vaccination requirement. Violation 2-112-340 no citation issued all support documents/signs were given to the manager, which he presently posted at the front door and inside the premises.
