2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site with valid original certificate posted for public view while preparing and serving tcs foods (sliced deli meat, italian beef, ground beef, nachoi cheese). Instructed to provide and maintain at least one on-site. Priority foundation violation. #7-38-012. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on site during the time of inspection. Noted approximately 70 small rodent droppings underneath all three chip racks in the grocery area of the store and in between shelving units in the prep area scattered on the floor. Instructed to remove, clean, and sanitize said areas. It is recommended to contact pest control for service. Priority foundation violation. #7-38-020(a). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cutting boards with dark stains and deep grooves. Instructed to replace and maintain surfaces that are easily cleanable

49. Non-food/food contact surfaces clean
- comments: observed not-in-use equipment in need of cleaning. (slicers, hot plates, toaster ovens. Instructed to clean and store properly.

51. Plumbing installed; proper backflow devices
- comments: unable to locate back flow prevention device at mop sink in rear facility. Instructed to provide.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed garbage receptacle without lid in toilet room. Instructed to provide.

55. Physical facilities installed, maintained & clean
- comments: observed floors throughout prep area covered in carpet pieces, noted broken and missing tiles on floor near 3 compartment sink as well as in grocery near ice machine and cooling units covered with carpets. Instructed to remove and provide smooth and easily cleanable floors and maintain.

57. All food employees have food handler training
- comments: observed no food handler certificates on site for review. Instructed to obtain.
