- comments: observed evidence of rodent activity on the premises. Observed approx. 10 mouse droppings behind the mini-fridge in the principal's office of room 117 and observed approx. 15 mouse droppings inside of the cabinets next to the door of room 134. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

54. Garbage & refuse properly disposed; facilities maintained
- comments: the outside garbage area is not maintained clean. Observed excessive litter and garbage on the ground under and around the garbage dumpsters. Management instructed to clean and maintain the outside garbage area at all times. Priority foundation violation 7-38-020(b).
