- comments: observed card board being used as a liner on shelves for food storage. Instructed to clean and remove all card board boxes to prevent hiding places for pests.

- comments: observed heavy grease build up inside of cooking fryer in kitchen prep area. Must clean and maintain. Also observed heavy ice build up in walk in freezer in rear. Must defrost to prevent contamination.

- comments: observed dust accumilation on vents in meat storage area in rear. Must clean and maintain.
