Spicy Chicken Sandwich Illness - Wendy's, Brigham City Utah

I ate a spicy chicken sandwich without the tomato but with lots of lettuce. 5 hours later I suffered severe intense symptoms checked off above. I am now at 16 hours later and most of symptoms have subsided or been much reduced. But at one point I felt like I needed to go to the hospital because I collapsed on the floor and was too weak to move. | Symptoms: Nausea, Diarrhea, Vomiting, Stomach Pain, Fever, Cramps, Chills
