- comments: observed no vomit and diarrhea clean up kit/procedure on site. Must provide written procedure for the clean up of vomit and diarrheal events, and corresponding equipment outline in procedure. Priority foundation violation 7-38-005. No citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed debris on floors throughout 2nd floor. Must elevate all items 6 inches from the floor and 6 inches from the walls to allow for proper cleaning.
59. Previous priority foundation violation corrected
- comments: observed previously issued priority foundation violations from report # 2382424 dated 6/29/20 not corrected. Must correct the following violations: #38 facility unable to provide proof of commercial pest control service. Instructed must provide most recent proof of service from a licensed, commercial pest control company. Priority foundation 7-38-020(c). No citation issued. Observed appx 50 small, black flies on walls and ceilings near 3 compartment sink and in rear of prep area near mop sink. Instructed must detail clean and sanitize these areas. Must contact pest control for service to minimize pest issue. Priority foundation violation 7-38-020(a). No citation issued. Observed approximately 25 mice droppings on floors on 2nd floor. Must remove, clean and sanitize. Priority violation 7-42-090. Citation issued. 7/31/20 observed appx 17 mice dropping scattered on floors throughout 2nd floor storage area. Observed approximately 3 live small flies in rear mop sink area. O observed approximately 20 dead bugs on floors throughout 1st and 2nd floor. Observed open granulars on stairs leading to 2nd floor. Not permitted. Must remove.
