- comments: evidence of live roaches on site. Observed 3 live baby 1 large roach under dishmachine,1 baby roach inside to-go paper bags container in dry storage area. 2 live roaches inside cardboard rodent bait boxes. Instructed to clean/sanitize all affected areas. Strongly recommend pest service. Priority foundation issued 7-38-020(a)

- comments: observed rear door inside of dry storage room not rodent proof at bottom areas. Must repair.

- comments: interior of coolers, shelves in rear prep area not clean of grease , debris. Instructed to clean/maintain.

- comments: observed missing floor tiles and floor need re-grouting under and around dish machine where there is stagnant water is on top of missing tiles areas. Instructed to replace and re-grout areas to prevent breeding areas for insects.
